What If I Could Improve Quality with Data Automation & Predictive Modeling?

Fifty percent of hospital acquired conditions are considered preventable and account for ¼ of inpatient costs. In addition, almost 30% of hospital readmissions within the first 30 days are considered preventable. CMS plans to reduce Medicare payments for preventable hospital readmissions by as much as $7B. Imagine the potential for saved lives and cost reduction driven by more efficient patient monitoring and readmissions management through automated data analysis and risk stratification. During this session, we will explore opportunities to prevent unplanned ICU transfers and proactively reduce hospital readmissions. Providence Medical Center will share how they have progressed from retrospective manual analysis to a proactive automated patient monitoring system call Modified Early Warning System (MEWS) to reduce ICU transfers. MedStar Health, Washington Hospital Center, will share how they are managing hospital readmissions through predictive modeling to risk stratify readmission probability at the time of inpatient discharge.

What If I Could Demonstrate Value and Improve Quality Through Performance Measures?

Enhancing the value of healthcare services through improving quality and controlling costs is a primary focus of health reform. The Affordable Care Act contains provisions related to setting priorities for improvement, measuring performance, establishing the platform for efficient data collection and information sharing, and using performance information in ways that promote the achievement of quality improvement goals. The various public and private sector entities that comprise the healthcare quality enterprise, including the National Quality Forum (NQF), are organizing to support the implementation of health reform. This presentation will highlight the quality aspects of health reform and describe NQF's role, particularly in convening multi-stakeholder groups to provide input to the Department of Health and Human Services on priorities for healthcare quality improvement and on the selection of performance measures for public reporting and performance-based payment incentives.

Best Practices for Clinical and IT Collaboration: A Case Study with Golden Living

The Golden Living family of companies is a collection of healthcare businesses, concentrated in skilled nursing, rehabilitation therapy, home health, hospice, healthcare staffing and purchasing solutions. Collectively, Golden Living has over 40,000 employees providing full-service recovery care to more than 60,000 patients everyday across 37 states. The organization's enterprise-wide IT strategy is built around using Amalga to improve quality of care and patient experience, increase employee satisfaction, and differentiate Golden Living services to its referring hospitals and physicians. Golden Living will describe how it's accomplishing reduction in re-hospitalization resulting from an increased ability to intervene early and an estimated 3,000+ hours a day saved on paperwork across the organization which results in increased hours of hands on patient care. The Impact of Health Information Technologies and Comparative Effectiveness Research on Care Improvement

The Institute of Medicine estimates that approximately one-third of health care spending involves procedures and treatments that neither extend life nor improve the quality of life. Often innovative care improvement strategies are put into "real-world" practice before sufficient evidence is collected to prove their clinical value, safety and effectiveness. 2009 ARRA funding, however, is creating opportunity to achieve timely, evidence-based improvements in care through the application health information technology (HIT) and comparative effectiveness research (CER).
