Microsoft HealthVault Community Connect is a patient and provider portal solution that enables healthcare organizations to easily generate electronic visit summaries using health industry standards (such as CCD or CCR) and share them with their patients and the provider community.

Connects your referral community
You’ll get help building longstanding relationships with referring physicians by improving the level of service your organization can provide to them. Referring doctors can access their patients’ visit summaries and electronic medical records online, reducing the need for paper, faxes, or the valuable time of qualified personnel, and thus reducing costs. HealthVault Community Connect streamlines communication to help ensure the right information gets into the hands of those who need it most, enhancing clinical decision making and simplifying coordination of care. As such, HealthVault Community Connect can be an important solution for your care coordination needs, and can play an important role to address broader health information exchange (HIE) needs.
Connects with your patients
HealthVault Community Connect simplifies registration, helps patients keep their information up to date, and minimizes the need for paper forms. Thanks to the native connectivity with Microsoft HealthVault, it also enables patients to register for their visits through the online patient portal, input their information once, and use it many times. Doing so improves patient satisfaction and supports data quality, which in turn shortens your revenue cycle.
The discharge process is also simplified. Patients can get an electronic copy of their visit summaries or medical records online and can view them at any time. This copy is automatically saved in the patients’ HealthVault account. If they choose to, patients can use HealthVault-enabled tools and devices to become more engaged in their health, and share their electronic health information with their care team.

The patient and provider portals offered in HealthVault Community Connect are based on familiar and proven technology like Microsoft SharePoint Server and Microsoft Amalga.
