Narrator always reads aloud the text in any part of the desktop you select. You can also have it read aloud what you type, as well as events that happen on screen (such as dialog boxes that appear).
In Microsoft Narrator, under Main Narrator Settings, select what you want Narrator to read aloud:

When the Narrator settings window is minimized to the taskbar, you can restore it by pressing ALT+TAB until Microsoft Narrator is selected, then release the keys.
