Demonstrator shows Microsoft Groove in action at East Kent
Microsoft’s UK healthcare team has built a PowerPoint demonstration to show how Microsoft Groove is helping to prevent unnecessary hospital admissions and improve the care of elderly people in East Kent.
Government statistics suggest that every day, 3,500 elderly patients remain in hospital unnecessarily. In most cases, this is because plans for their ongoing care have not been finalised."Microsoft has created a virtual demonstration to show how healthcare professionals in East Kent can now use Groove, together with the standardised, evidence-based assessment tool called InterRAI, to record details about new patients and co-ordinate their care."
Eastern and Coastal Kent Primary Care Trust is dramatically improving patient care planning using a collaborative solution based on Microsoft Groove. At the heart of the solution is a collaborative assessment tool, developed with Microsoft and certified Microsoft partner D2i.
Microsoft has created a virtual demonstration to show how healthcare professionals in East Kent can now use Groove, together with the standardised, evidence-based assessment tool called InterRAI, to record details about new patients and co-ordinate their care.
Fewer delays, more information sharing
The demonstration shows that using the application means that patients no longer face unnecessary delays while waiting for multi-disciplinary teams to organise assessments. Nor do they need to answer the same questions about their condition over and over, because members of the team can quickly and easily share information using a secure patient workspace within Groove. "The demonstration shows how authorised professionals from multiple agencies can share documents and exchange messages securely, and see instantly when a message is received or a request is actioned."
Better integration, better communication, better care
Groove isn’t just a way for multi-disciplinary teams to share assessment data, however. The demonstration shows how health and social care professionals can work together within Groove to build an integrated care plan with goals, tasks and outcomes for each patient. Notes and subsequent assessment requests can all be made within Groove, and shared with all relevant parties.
A patient’s workspace is also a virtual repository for all kinds of information relevant to their care. The demonstration shows how authorised professionals from multiple agencies can share documents and exchange messages securely, and see instantly when a message is received or a request is actioned.
In this way, Groove provides a secure, efficient alternative to conventional email or telephone messages.
