Streamlining the handover of care from hospital to community providers with better information.
Underlying the new rules, which come into effect this April, requiring hospital-discharge letters and summaries to be delivered to GPs within three days of a patient leaving hospital, there are strong drivers for hospitals to switch from manual to electronic systems.
Current patient-discharge practices have been identified as a specific barrier in ensuring patient safety; also in making the patient experience seamless, regardless of organisational boundaries. Furthermore, inter-hospital competition for patients means many clinical directorates need to maximise their efficiency and throughput.
This event offers a series of presentations and case histories that will give you an up-to-date picture on the practicalities of adopting electronic patient-discharge summaries. You will gain a clearer understanding of the information needs of everyone involved in this part of the patient journey and learn how best to meet them; and you'll be able to explore leading solutions at the accompanying exhibition.
