[music] IHiS works with the Ministry of Health leveraging cost-effective technology to support over 50,000 health care users. Our goal is to build a resilient and intelligent health administration system for Singapore. During the height of the pandemic in May 2020, we needed a way to reduce the crowd gathering at the public health care or community care facilities. So we came up with an idea where patients took ownership of their health by self-tracking their vitals like temperature, blood pressure, and oxygen saturation during their time in isolation. Later on, clinicians can monitor this vital submission remotely to do a clinical intervention. Therefore, a platform was created called "Health Discovery", which is really about vital sign monitoring. Or we just simply call it HD VSM. [music] We realized HD VSM was applicable beyond COVID-19; thus HD+ was created. The core idea is still about empowering patients support their primary and post-discharge care needs like basic hypertension, cardiac rehabilitation, diabetes management, and so on. [music] We really need something agile to set everything quickly, yet scalable, while not compromising the security aspect of our platform. And with many technology stack of IHiS built on Microsoft, this made Microsoft Azure an obvious choice to deploy our solution. [music] So HD+ improves efficiency. So the patients submit their vital signs via the mobile app, and we are able to see them on our clinician dashboard and appears color-coded in terms of the priority and urgency. Another feature allows us to survey patients via the mobile app, and the required data appears on our dashboard. HD+ doesn't just help patients monitor vital signs, but also sends important reminders via mobile notifications. [music] Overall, HD+ helps community nurses to remotely monitor residents with hypertension and eliminates the manual transcription of BP readings into our medical records. This frees up the time for community nurses to focus on residents who require more care and support in the community. [music] As of end May 2022, 80 enrolled residents had abnormal BP readings. The HD+ dashboard flagged them up to our community nurses, who promptly followed up with the residents. [music] More than 1,000 patients use HD+ across 16 polyclinics to monitor their blood pressure using HD+ mobile app. More than 160,000 vital signs have been collected with 11,000 identified by HD+ for clinical intervention to date. [music] Our next priority is expanding to other use cases by developing what we call "clinical protocols". At the end of the day, we want to help as many patients as we possibly can for a healthier nation. [music]
